Laparoscopic Management of Gastroesophageal Reflux Disease in Neurologically Impaired Children

Abstract
Gastroesophageal reflux (GER) is the backward movement of acid-containing gastric contents into the esophagus, and gastroesophageal reflux disease (GERD) is defined as chronic symptoms and/or associated complications produced by GER. There are several treatment options for pediatric GERD, such as lifestyle modifications, pharmacologic therapy and surgical treatment. Neurologically impaired children comprise a large proportion of pediatric patients who require surgery for medically intractable gastroesophageal reflux. Laparoscopic Nissen fundoplication combined with gastrostomy were effective for pediatric GERD, even in children with serious neurological deficits. However, the specific technique used for adults may not always be appropriate for children, especially those with neurological impairment. The authors report three cases of GERD in disabled children that were successfully treated with laparoscopic Nissen fundoplication and gastrostomy. Similar cases have not been found in the Korean surgical literature.
